REGISTRATION

CESA #12 SCHOOL DISTRICTS	NON-CESA #12 SCHOOL DISTRICTS
Early Registration/Prior to May 15, 2008 = $350  Late Registration/After May 15, 2008 = $450	Early Registration/Prior to May 15, 2008 = $400  Late Registration/After May 15, 2008 = $500

A district check, purchase order, or a personal check, must accompany the registration form.  Participants are responsible for their own lodging, travel, and meals other than breakfast and lunch.

CESA #12 MATH/SCIENCE CONSORTIUM MEMBERS (DRUMMOND, HURLEY & SOUTH SHORE)
          Receive 3 pre-paid registrations to the Academy.

CESA #12 TITLE IID CONSORTIUM MEMBERS (BUTTERNUT, GLIDDEN, HURLEY, MELLEN, MERCER, PARK FALLS & WASHBURN)
          Receive 2 pre-paid registrations to attend any technology workshop during the 2008 Academy.

CREDIT

Participants will have the option of receiving 1 to 2 graduate credits from Northland College for $220 per credit.  Applications for credit will be provided with your confirmation letter and should be mailed to Northland College prior to the start of the Academy.

2-Day Workshops = 1 Graduate Credit
3-Day Workshops = 1 Graduate Credit
5-Day Workshops/1 or 2 = Graduate Credits

An additional 1-2 credit(s) will be available for participants of the Academy.  This additional credit option will be available during the school year through Northland College.  The fee for this credit will be $220 per credit.

CANCELLATION POLICY

Refunds for cancellation will be made if written request is received via fax or mail by June 23, 2008.  No refunds will be made after June 23rd.  If a registrant has not cancelled, but does not attend the Academy, she/he will be held responsible for payment.

SESSIONS NOT RECEIVING SUFFICIENT ENROLLMENT BY MAY 15, 2008 MAY BE CANCELLED.
